CERTIFICATES/DOCUMENTS REPLACEMENT POLICY
1. Purpose
This policy outlines the procedures and requirements for replacing a lost or damaged certificate, including the payment of a fee for the issuance of a new certificate or documents.
2. Scope
This policy applies to all certificates and documents issued by our INSTITUTIONS, SCHOOLS, AND ORGANIZATIONS including but not limited to:
- Professional licenses
- Academic credentials
- Membership certificates
- Certifications
3. Eligibility for Replacement
A certificate may be replaced if it is:
- Lost
- Damaged or torn
- Stained or faded
- Incorrectly issued
4. Procedure for Replacement
To request a replacement certificate or documents, the individual must:
- Submit a written request to the relevant department or email.
- Provide proof of identity (e.g., government-issued ID).
- Pay the applicable fee for the replacement certificate.
- Submit any additional documentation required by the organization.
5. Fee Structure
A fee will be charged for the issuance of a new certificate. The fee amount is determined based on the type of certificate and the cost of production. The fee is non-refundable and must be paid in full at the time of application.
6. Processing Time
The organization will process the replacement request within 5 to 14 business days after receipt of all required documentation and payment.
7. Exemptions
In cases of emergency or hardship, the organization/ institution may consider waiving or reducing the fee upon approval by the relevant authority.
8. Responsibility
The individual is responsible for ensuring the accuracy of all information provided and for the validity of the certificate.
